Thank you very much for the report and the recording, it was exactly what I needed.
I found the cause. When you change chords with the pedal (C → Dm → G/E → G/F), PianoToLife takes over the previous bass notes with its own strings, using partials measured from your piano's sound. A new chord struck right next to one of those partials could leak into that measurement: in your recording, the B and G of the G/E chord made two high harmonics of the D in the bass much too loud. They then rang as two pure tones "between the notes", which is the strange extra note you heard. It could happen more or less at random, depending on the chords.
The fix is done and is being tested. It will be in PianoToLife 9.0.4, together with some interface polishing. I'll post here as soon as it's out.
